Practice Transformation Advisor

We are seeking a highly motivated and experienced Practice Transformation Advisor to join our dynamic team.
As a Practice Transformation Advisor, you will play a critical role in ensuring the highest standards of performance and quality within our organization's network of healthcare providers.
Your primary focus will be to identify areas for quality improvement, implement quality strategies to enhance provider performance, and drive positive outcomes in the delivery of patient care.
Documenting financial and business transformation requirements Implement Finance process transformation and improvements Navigate change and enroll people throughout the transformation continuum Train a dedicated project team during four-month transformation projects Manage and train a dedicated project team during four-month transformation projects Lead multi-disciplined practice transformation and practice performance improvement initiatives Work closely with manager and stakeholders to deliver on transformation opportunities via targeted projects Support providers with transformation to health homes or PCMH models Maintain relationship with the practice leadership and key clinical influencers actively involved in practice transformation Help coordinate across various initiatives under cost transformation and ensure all deadlines are met Conduct regular progress reviews to ensure sustainability Implement and sustain transformation activities and process improvements Providing just in time coaching on business transformation and process improvement methodologies and tools Coordinate resources to achieve project goals Provide operations support for transformation efforts including change readiness, development, communications, implementation, metrics and sustainability Assessing future cost savings opportunity potential and process improvement initiatives Help through the activities needed for business transformation Implement appropriate performance improvement initiatives designed to assist the practice in achieving contractually required transformation milestones Work directly with senior-level leaders across functions and geographies to identify and drive transformation throughout organization Identify new business opportunities as well as consult on existing accounts and seek to create, develop, and strengthen strategic client relationships with key program sponsors o Bachelor's degree or commensurate work experience in healthcare administration, public health, or a related field.
o 3-5 years of experience in healthcare quality improvement, provider relations, or a related role.
o In-depth knowledge of healthcare regulations, clinical operations, quality standards, and performance metrics.
o Strong analytical and problem-solving skills, with the ability to interpret complex data sets and identify improvement opportunities.
o Excellent communication, presentation and interpersonal skills to collaborate with and effectively influence provider groups and executives, , team members, and stakeholders at all levels.
o Proven experience in designing and delivering training programs or educational initiatives.
o Proficiency in data management and analysis tools, such as Excel or data visualization software.
o Familiarity with electronic health record systems and healthcare information technology.
o Detail-oriented, organized, and able to manage multiple projects simultaneously.
o Ability to work independently, demonstrate initiative, and drive results in a fast-paced environment.
o Position is remote - secure home network required and familiar with Microsoft Office products and VPNPreferred:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
